BECAUSE…

There is no one else to do what I do.

These questions regarding overall emotional health are on a Screening Questionnaire for an Annual Physical.

Over the past two weeks, how often have you been bothered by the following problems?

NOT AT ALL       SEVERAL DAYS           MORE THAN HALF THE DAYS          NEARLY EVERYDAY

Little interest or pleasure in doing things.

Feeling down, depressed, hopeless

Trouble falling or staying asleep or sleeping too much

Feeling tired or having little energy

Feeling bad about yourself or that you are a failure or have let yourself or your family down

Trouble concentrating on things such as reading the newspaper or watching television

          Nearly Everyday

Moving or speaking so slowly that other people could have noticed. Or the opposite-being so fidgety or restless that you have been moving around a lot more than usual

          Not at All

Thoughts that you would be better off dead or of hurting yourself in some way

          Not at All

Feeling nervous, anxious or on edge

          Nearly Everyday

Not being able to stop or control worrying

          Nearly Everyday

Worrying too much about different things

          Nearly Everyday

Trouble relaxing

          Nearly Everyday

Being so restless that it’s hard to sit still

          Not at All

Becoming easily annoyed or irritable

          Nearly Everyday

Feeling afraid as if something awful might happen

          More than Half the days

If you checked off any of the problems above how difficult have these problems made it for you to do your work, take care of things at home or get along with other people?

NOT DIFFICULT AT ALL         SOMEWHAT DIFFICULT           VERY DIFFICULT                  EXTREMEMLY DIFFICULT

           Not Difficult at All. 

As a Caregiver, although these problems may occur Nearly Every Day, they do not interfere with my Caregiving responsibilities—Not at All!

In Contrast, my Mother’s answers to those exact questions in every case was, “Not at All.”
